Insurance Coverage

Participation and benefits vary by plan, product, service and appointment modality. Before booking, contact our office to confirm current participation for your specific plan.

You should also verify mental-health and telehealth benefits, deductibles, copays, coinsurance and other out-of-pocket terms directly with your insurer. Information from the office is not a guarantee of payment by the insurer.

Copayments

Your insurer and the office can confirm any copayment, its timing and the payment methods currently accepted. Request the applicable terms in writing before payment.

Self-Pay Patients

Contact the office for current written self-pay fees. A Good Faith Estimate is provided when required by applicable law; ask whether it applies to the proposed service.

Late Cancellation Fees

The current written Cancellation Policy governs. Ask the office to confirm any applicable fee in writing before scheduling.

Payment Methods and Processing Fees

Ask the office to confirm in writing the payment methods currently accepted and any processing fee that may apply. Any fee remains subject to disclosure requirements and applicable law.

Insurance Claims Processing

If the office confirms that a claim can be submitted for your plan and service, processing and payment decisions remain subject to the insurer's rules. Any patient responsibility will be addressed according to the applicable plan information and billing statement.

Billing Arrangements
