Telehealth Informed Consent

This document explains how telehealth works at Healing Minds Psychiatry and describes its benefits, limitations, and risks so that you can make an informed decision about your care. Please read it carefully before your visit.

Healing Minds Psychiatry offers telepsychiatry appointments when the office and treating professional confirm that this modality is appropriate. Telehealth uses a real-time video platform rather than an in-person visit. Platform, privacy and access details are subject to the practice Privacy Notice and the limitations described below.

Legal Basis for Telehealth

Telehealth consent and documentation are handled under applicable law, including Florida Statutes Section 456.47 and California Business and Professions Code Section 2290.5 when applicable. The treating professional must be authorized to practice where the patient is physically located, and the office confirms eligibility before the appointment. A website request does not by itself establish a provider-patient relationship or replace documented clinical consent.

Potential Benefits

• Access: A confirmed video visit may reduce the need to travel to an office.
• Convenience: A video appointment may offer flexibility when clinically and operationally appropriate.
• Follow-up: Telehealth follow-up may be offered when the treating professional considers it appropriate.
• Language: English or Spanish services may be requested; the office confirms availability.

Limitations and Risks

Telehealth may be appropriate for some visits and has limitations and risks that you should understand:

• No complete physical examination: A telehealth visit does not allow for a full, hands-on physical examination.
• Technology failures: Connections may fail or be interrupted, and technical problems can affect the timing or quality of a session.
• Privacy and security risks: In rare cases, electronic transmissions carry a risk to privacy or security despite the safeguards we use.
• Appropriateness of care: Telehealth may not be appropriate for every condition. Your provider may determine that in-person care or a higher level of care is needed and will help you arrange appropriate alternatives.

Not for Emergencies

Telehealth is not an emergency service. If you or a loved one is experiencing a life-threatening crisis, call 911 or call or text the 988 Suicide and Crisis Lifeline immediately. Our Emergency and Crisis Policy applies to all telehealth care and explains additional resources available to you.

Your Responsibilities

• Location: You agree to state your physical location at the start of each session. The treating professional must be authorized to practice in that location, and the office confirms eligibility before the appointment.
• Identity verification: You agree to verify your identity as requested at the beginning of your appointment.
• Private setting: You are responsible for finding a private, quiet space where your session cannot be overheard.
• Adequate connection: You are responsible for having a device and an internet connection adequate for a video visit.

Health information is handled under applicable privacy law and the practice Privacy Notice. The practice uses reasonable administrative and technical safeguards, but electronic communications carry privacy and security risks. Ask the office about the current platform and recording practices before consenting.

Prescriptions

Prescriptions are issued only when clinically appropriate and only when permitted by applicable state and federal law. Attending a telehealth visit does not guarantee that any medication will be prescribed. All treatment decisions remain a matter of clinical judgment.

Your Rights

You may withdraw your consent to telehealth at any time and discuss alternatives with your provider. Withdrawing consent to telehealth does not affect your right to future care.

Acknowledgment

Before telehealth care begins, the treating professional obtains and documents consent as required by applicable law. Scheduling or submitting a website request does not substitute for that clinical consent.
